COOPERATIVE POLICIES

I pledge to respect the policies and regulations of the “ Coopérative de Solidarité Santé Globale Saint-Adolphe- d’Howard” (COOP-SANTÉ) and to pay the annual contribution.  

Upon my resignation, I will donate my shares to the Cooperative should I refrain from submitting a written refund claim. Note : For User members, each adult of the same family must complete an application and pay the registration fee.  

Protection of personal information  The COOP-SANTÉ agrees, without time limit, to protect and to maintain the 
strictest confidentiality concerning personal information.

Registration (share)
A registration fee of $50.00 is required in order to become a member. Becoming a member does not ensure that you will be assigned a family doctor. 

The registration fee is payable one time only and is refundable, by written request only, should you no longer wish to be a member and there is no balance due to the Cooperative.

A member is presumed to be inactive if, during the past 3 years, he/she has ceased to do business with the Cooperative as per article 38.1 of the cooperative law. 

Annual Contribution
The annual contribution covers the period of July 1st  to June 30th .  Kindly remember that this contribution enables the Cooperative to maintain its operations (rental of our location, equipment & staff).  

The invoice for the annual contribution is payable upon receipt.  The contribution is subject to change and is determined by the administration council each year.  

A member who registers between April 1st and June 30th , will be invoiced for the annual contribution.  This will therefore cover the period of registration up to the 30th of June of the following year.  

Default of the annual contribution payment
A default in the payment of the annual contribution, of 2 years or more, may lead to the cancellation of the membership.  

As per article 27 of the cooperative law, if a balance is due, the registration fee ($50.00) will be applied towards the balance due and you will no longer be a member of COOP-SANTÉ.  In order to become a member again, you will need to re-apply as per our current registration policy. 

General Information
Our doctors operate by appointment only. To make an appointment, please contact us by telephone at 819-327-3534.

Cancellation of appointment 
To cancel an appointment, kindly contact our office 48 hours in advance of the appointment.

For ANY patient who does not present himself/herself at their scheduled appointment, and who omits to advise our office that they are unable to attend, a fee of $30.00 will be applied and an invoice will be mailed to you.

Reports and miscellaneous 
An administration fee of $25.00 will be applied for any NSF (non-sufficient funds) check received. An invoice will be mailed to you.

Fees are applicable for completion of all medical reports.  A schedule of applicable fees can be found displayed in the waiting area of Coop-Santé which respects the provincial grid as established by the RAMQ (medicare).  You may pay these fees by check or cash only at your scheduled appointment.

Fees are also applicable for prescription renewal requests made without a medical appointment.

Fees of $50.00 to $100.00 may be charged to patients who do not possess a valid medicare card at the time of their appointment. These fees are payable directly to the doctor and cash only will be accepted.

We reserve the right to refuse a member or to reimburse an already registered member, for any inappropriate or disrespectful behaviour towards our staff or our procedures.
