Dear patients,

Your family doctor, member of the Coop santé St-Adolphe and Morin-Heights, invites you to register on our new PATIENT-PORTAIL.

This portal makes communications easier and allows you to consult, download and print results that will be shared with you by your doctor. This communication will be ONE-WAY; you will therefore not be able to communicate with your doctor through the patient portal.

In the near future, this portal will also allow you to schedule appointments on line. A new communication will be sent to you when this service is available.

You can proceed now to the creation of your patient portal. The activation of it will be completed by our team during your next appointment.

·         Each patient will need a personnel email-address.

·         To register your children (< 14 yrs old), you will absolutely have to go through the secretariat on site. This option is not available by the link.
